• Date of Last Charge
• Amount of Last Charge

Follow these steps:

1. Goto the website https://help.lumosity.com/hc/en-us/requests/new
2. Pick “Refund Request”
3. Click the link that reads “Continue without Logging In”
4. Now put in your info where it asks for it and ask to cancel within the body of your message
5. Click the link that reads “Submit” and wait for a response via email
6. As usual, just in case, keep the proof that you’ve cancelled handy.
